Definition
This is the patient who over responded (collection of > 15 eggs) to stimulation in a previous IVF cycle. If there is no history of previous treatment, she is typically young and has polycystic ovaries.
Treatment Protocol
If the patient is not menstruating regularly induce a withdrawal bleed with MPA (progesterone tablets for 7-10 days). Ask the patient to come for an ultrasound on the 3rd day of her cycle. If there are no follicles > 10 mm in size and if the endometrium is thin start stimulating the ovaries. The typical dose is 150-225 IU of Menogon/Gonal-F/Puregon. If the patient is lean give 150 IU if she is of normal weight give 225 IU and if she is obese give 300 IU. After 5 days of stimulation send the patient to the American Hospital for ultrasound and follow-up of the treatment.
